How to get started

WIC program, or Women, Infants, and Children program, is a federal assistance initiative that provides nutritional support to low-income pregnant women, new mothers, and young children. It offers access healthy foods, nutrition education, and healthcare referrals help ensure the well-being of families. 

To apply for our WIC program, please follow these steps:

1. Collect all necessary documents.

a. Identification proof.

b. Address verification.

c. Income verification (Medicaid ID, HRA case number, or pay stubs to assess eligibility).

d. WIC Medical Referral forms or recent height and weight records from your healthcare provider (such as a summary visit or vaccination card). If you are pregnant, include proof of pregnancy.

2. Call us at (718) 857 - 4268  or schedule your appointment online.
